📋 Description

• Head-to-toe assessments; identify subtle changes in patient condition
• Acute and post-acute care experience (CHF, COPD, wound care, IV therapy)
• Triage and manage urgent clinical situations independently at home
• Medication reconciliation, patient teaching, safety monitoring
• Wound assessment, care, and documentation
• Flexible, adaptable to changing patient needs and schedules

🎯 Requirements

• Current nursing license in good standing in Arizona
• Graduate of accredited nursing program
• At least 1 year of adult/geriatric clinical experience in acute care, home health, or similar
• Ability to work independently and make real-time clinical decisions
• Strong problem-solving in unpredictable home environments
• Manage dynamic visit schedules; prioritize urgent visits
