Registered Nurse -RN Home Health

📋 Description

• Provide skilled nursing and rehabilitative care at patients' homes.
• Perform head-to-toe assessments and identify subtle changes in condition.
• Care for acute and post-acute conditions (CHF, COPD, wound care).
• Triage and manage urgent clinical situations independently in the home.
• Medication reconciliation, teaching, and safety monitoring.
• Wound assessment, care, and documentation.

🎯 Requirements

• Current Vermont nursing license in good standing.
• Graduate from an accredited nursing program with transcript or diploma.
• At least 1 year of hands-on adult/geriatric experience in acute care, ER, home health, or rehab.
