📋 Description

  • Head-to-toe assessment to detect changes in patient condition
  • Acute and post-acute care: CHF, COPD, wounds, IV therapy
  • Triage and manage urgent home clinical situations independently
  • Medication reconciliation, teaching, and safety monitoring
  • Wound assessment, care, and documentation

🎯 Requirements

  • Current Vermont nursing license in good standing
  • Graduate of an accredited nursing program (transcript or diploma)
  • At least 1 year of hands-on experience with adult/geriatric patients in relevant settings
