Registered Nurse - RN Home Health

📋 Description

• Full-time RN to join Vermont home health teams.
• Provide skilled nursing and rehabilitative care at clients' homes.
• Work with a multidisciplinary team to keep patients at home.
• Opportunities across Vermont with needs in Windsor, Orange, Windham.
• Support patients with head-to-toe assessments and condition monitoring.
• Coordinate care and prioritize patient safety.

🎯 Requirements

• A current Vermont nursing license in good standing.
• Graduation from an accredited nursing program with transcript or diploma.
• At least one year of adult/geriatric experience in relevant settings.
• Ability to work independently and escalate concerns as needed.
• Comfort with real-time clinical decisions without on-site support.
• Strong problem-solving in unpredictable home environments.
