📋 Description

• Head-to-toe assessments; identify subtle patient condition changes
• Deliver acute and post-acute care (CHF, COPD, wounds, IV therapy)
• Triage and independently manage urgent clinical situations at home
• Medication reconciliation, teaching, and safety monitoring
• Wound assessment, care, and documentation
