📋 Description

• Provide skilled nursing and rehab care at patients' homes.
• Travel across the service area to visit patients.
• Perform head-to-toe assessments; identify subtle changes.
• Manage acute and post-acute care (CHF, COPD, wounds, IV).
• Triage urgent clinical situations independently in the home.
• Reconcile meds, teach patients and monitor safety.

🎯 Requirements

• Current Colorado nursing license in good standing.
• Graduate of an accredited nursing program (transcript/diploma).
• At least one year of adult/geriatric clinical experience.
• Willingness to travel throughout the coverage area.
• Ability to make real-time clinical decisions independently.
• Prior Medicare Home Health experience preferred.
