📋 Description

• Conduct head-to-toe assessments and spot subtle condition changes.
• Provide acute and post-acute home health care.
• Triage and independently manage urgent clinical situations at home.
• Perform medication reconciliation, teaching, and safety monitoring.
• Complete comprehensive wound assessment, care, and documentation.

🎯 Requirements

• Current Colorado nursing license in good standing.
• Graduation from an accredited nursing program.
• At least one year of adult/geriatric clinical experience.
• Ability to work independently and escalate as needed.
• Make real-time clinical decisions without on-site support.
• Manage a dynamic visit schedule and urgent priorities.
