Registered Nurse, RN - Home Health Visits

📋 Description

• Perform home health visits for adult and geriatric patients in designated SC counties.
• Follow a patient-specific care plan and document observations on a touch pad tablet.
• Deliver medication administration, wound care, treatments, and procedures as prescribed.
• Monitor patient conditions and report changes to the Clinical/Client Services Manager.
• Coordinate case management and follow doctor orders with proper documentation.
• Offer 1:1 care with flexible schedules and opportunities for advancement.

🎯 Requirements

• Current South Carolina RN license.
• Minimum one year of recent medical/surgical clinical experience.
• Graduate from an accredited nursing program (transcript or diploma).
• Ability to work independently and manage time effectively.
• Strong interpersonal skills; proficient EMR experience preferred.
• Ability to travel to cases within territory.
