Provide home health services to adult and geriatric clients on a per visit basis
Make home visits in Pitt and Martin counties
Follow designated care plans and document observations
Perform meds administration, wound care, treatments as assigned
Monitor client conditions and report changes to supervisors
Coordinate care and document all activities using EMR tools

🎯 Requirements

Current North Carolina RN license
Minimum one year clinical experience
Home care experience preferred but not required
Graduation from accredited nursing program
Ability to work independently and manage time
Strong interpersonal and computer skills with EMR familiarity
