Registered Nurse, RN, Home Health Visits (Med Surg + Wound Care)

📋 Description

• Follow designated care plan per patient needs.
• Make home visits to clients in Western Cuyahoga County.
• Perform meds, wound care, treatments, and procedures.
• Monitor client conditions and report changes to supervisor.
• Document assessments, interventions, and care in EMR on a tablet.
• Travel to cases as assigned; 1:1 care during visits.

🎯 Requirements

• Current OH Registered Nurse license.
• At least 1 year Med/Surg clinical experience.
• Graduation from accredited nursing program.
• Home care experience preferred but not required.
• Ability to work independently and manage time.
• Strong interpersonal skills, EMR experience, and computer skills.
