Registered Nurse, RN Preceptor

📋 Description

• Full Time Registered Nurse (RN) Preceptor – train, coach and mentor newly hired RNs and LPNs.
• Travel throughout the office’s coverage area to support onboarding activities.
• Maintain caseload and patient care responsibilities while onboarding others.
• Collaborate with a multi-disciplinary team to provide skilled nursing and rehab care at home.
• Support adult and geriatric patients in their homes around Wilmington, MA area.
• Medicare Certified Home Health and OASIS documentation experience required.

🎯 Requirements

• A current nursing license in good standing in MA.
• Graduation from an accredited nursing program.
• At least one year of adult/geriatric clinical experience in relevant settings within past two years.
• Experience with acute and post-acute care; CHF, COPD, wound care, IV therapy, post-surgical care.
• Strong ability to triage, manage urgent clinical situations, and perform medication reconciliation.
• Excellent documentation and time-management skills; flexible with travel and home visits.
