Registered Nurse, RN - Senior Living Visits

📋 Description

• Conduct head-to-toe assessments and detect subtle changes
• Deliver acute and post-acute care (CHF, COPD, wound care, IV therapy)
• Triage and manage urgent clinical situations at home
• Perform medication reconciliation, patient teaching, safety monitoring
• Conduct comprehensive wound assessment, care, and documentation
• Provide care within senior living communities in Melrose, Swampscott, and surrounding areas

🎯 Requirements

• Massachusetts nursing license in good standing
• Graduation from accredited nursing program
• At least 1 year experience with adult/geriatric populations
• Ability to work independently in home health
• Strong clinical decision-making and problem-solving
• Excellent communication and documentation skills
