Registered Nurse - Senior Living Visits

📋 Description

• Conduct head-to-toe assessments to detect subtle changes
• Manage acute and post-acute care needs (CHF, COPD, wounds, IV, post-op)
• Triage and manage urgent in-home clinical situations independently
• Perform medication reconciliation, teaching, and safety monitoring
• Perform comprehensive wound assessment, care, and documentation
• Collaborate with multi-disciplinary teams in Senior Living Communities

🎯 Requirements

• Current nursing license in PA in good standing
• Graduate of an accredited nursing program
• At least 1 year of adult/geriatric clinical experience
• Ability to work independently with escalation when needed
• Comfortable making real-time clinical decisions in home settings
• Flexible and adaptable to changing patient needs
