📋 Description

• Lead, mentor, and support a team of physical, occupational, and speech-language therapists
• Provide clinical and operational oversight for home-based rehabilitation services
• Support high-quality, coordinated therapy care from referral through discharge
• Partner with therapists to promote individualized treatment planning and meaningful client outcomes
• Support care coordination among therapists, nurses, physicians, clients, and families
• Review documentation, care plans, and service delivery for accuracy, timeliness, quality, and

🎯 Requirements

• Current professional license in Physical Therapy or Occupational Therapy
• Experience providing rehabilitation services in home health or related clinical setting
• Strong clinical judgment, organization, communication, and problem-solving skills
• Ability to support coordinated, outcomes-focused care across interdisciplinary team
• Comfort with electronic documentation and communication systems
• Ability to travel throughout service area
