Rehabilitation / Clinical Manager (Home Health)

📋 Description

• Provide direct supervision and support to field clinical staff
• Ensure compliance with Medicare Conditions of Participation (CoPs), state regulations, and agency
• Monitor and manage quality assurance processes, including OASIS documentation, Plan of Care
• Facilitate staff orientation, education, and performance evaluations
• Collaborate with the interdisciplinary team to ensure coordinated, efficient, and patient-centered
• Participate in the hiring, training, and retention of qualified clinical staff

🎯 Requirements

• Current license in good standing, Physical Therapist (PT) or Registered Nurse (RN)
• 2 or more years of experience in a leadership/ supervisory role
• Excellent organization and communication skills
• Desire to lead, inspire, and motivate others
• Strong knowledge of home health policies and procedures
• Demonstrated ability to manage clinical operations and lead teams through change
