Rehabilitation Manager, Home Health

📋 Description

• Lead, mentor, and support a team of physical, occupational, and speech-language therapists
• Provide clinical and operational oversight for home-based rehabilitation services throughout Ocean
• Support high-quality, coordinated therapy care from referral through discharge
• Partner with therapists to promote individualized treatment planning and meaningful client outcomes
• Support care coordination among therapists, nurses, physicians, clients, families, and other
• Review documentation, care plans, and service delivery for accuracy, timeliness, quality, and

🎯 Requirements

• Current professional license or credential in an applicable rehabilitation discipline, such as
• Experience providing rehabilitation services in home health, community-based care, or a related
• Strong clinical judgment, organization, communication, and problem-solving skills
• Ability to support coordinated, outcomes-focused care across an interdisciplinary team
• Comfort using electronic documentation and communication systems
• Ability to travel throughout Ocean County and surrounding communities as needed
