Rehabilitation Manager, Home Health Care

📋 Description

• Provide rehabilitation leadership and operational oversight for the MEF Senior Living Home Health
• Partner with the Director and local CSM/CM office team to support clinical quality, service
• Help ensure rehabilitation workflows, office priorities, and operational processes are executed
• Work independently to manage assigned responsibilities, solve problems, and follow through on
• Support clinicians through coaching, collaboration, and field-based engagement as appropriate to
• Build strong, respectful relationships with clients, families, employees, referral partners

🎯 Requirements

• Current, valid Pennsylvania license as a Physical Therapist, Occupational Therapist, or
• Demonstrated ability to work independently, manage competing priorities, and take ownership of
• Strong clinical judgment, communication, organization, and relationship-building skills.
• Ability to collaborate effectively with a multidisciplinary team in an office-based and
• Willingness and ability to travel throughout Delaware County and Center City Philadelphia as needed.
