RN Care Navigator/Case Manager

📋 Description

• Manage a caseload of members with varying levels of clinical complexity, conducting appropriate
• Conduct inbound and outbound calls to engage members, provide education, and deliver a high-quality
• Educate patients about kidney health, related conditions, dialysis options, transplant pathways
• Complete health and social determinants of health screenings, medication reconciliation, care
• Advocate for patients by identifying and resolving concerns, connecting them with appropriate
• Coordinate care as part of an interdisciplinary team, serving as a liaison between patients

🎯 Requirements

• Active, unrestricted Registered Nurse (RN) license required.
• Bachelor’s degree required; candidates with 6+ years of RN experience, including at least 3 years
• At least 3 years of case management experience preferred.
• Certified Case Manager (CCM) certification preferred.
• Experience working in a dialysis or transplant center is preferred.
• Familiarity with Medicare and Medicaid is preferred.
