RN Case Manager

📋 Description

• Supervise, mentor, and ensure the competency of assigned hospice care team
• Serve as team leader for patient and family coordination
• Oversee care plans, ensuring accurate evaluations, appropriate treatments, and optimal outcomes
• Monitor and review patient care for compliance with Medicare and hospice regulatory standards
• Review and approve care plans in EMR system, ensuring accuracy and adherence to Medicare Conditions
• Participate in quality assurance initiatives and performance improvement

🎯 Requirements

• Bachelor's degree or higher in health-related field
• Minimum 3 years clinical RN experience, with hospice experience required
• At least 1 year supervisory or management experience in hospice setting
• Strong knowledge of Medicare Conditions of Participation, Local Coverage Determinations, and
• Proficiency with Outlook, Word, and Excel
• Valid RN license in state of practice
