📋 Description

• Supervise, mentor, and ensure the competency of your assigned hospice care team, maintaining high
• Serve as the team leader, providing direction and oversight to interdisciplinary team members.
• Oversee care plans, ensuring accurate evaluations, appropriate treatments, and optimal patient
• Monitor and review patient care, ensuring compliance with Medicare and hospice regulatory standards.
• Review and approve care plans within the Electronic Medical Records (EMR) system, ensuring accuracy
• Participate in agency quality assurance initiatives, reviewing outcome data and driving performance

🎯 Requirements

• Bachelor’s degree or higher in a health-related field.
• Minimum of 3 years of clinical RN experience, with hospice experience required.
• At least 1 year of supervisory or management experience in a hospice setting.
• Strong knowledge of Medicare Conditions of Participation, Local Coverage Determinations, and
• Proficiency with Outlook, Word, and Excel.
• Valid RN license in the state of practice.
