Lead, inspire and motivate clinical staff to provide outstanding patient care
Conduct clinical assessments and supervisory visits throughout Fort Wayne and surrounding areas
Develop individualized care plans and oversee implementation of services
Supervise and support RNs, PTs, OTs, SLPs and other field staff
Facilitate new hire orientation and onboarding
Conduct competency evaluations for both new and tenured staff

🎯 Requirements

Minimum of 2 years experience as an RN
Prior Home Health and OASIS experience required
Acute care, medical-surgical, and/or sub-acute experience a plus
Strong clinical, communication, and organizational skills
Ability to build strong relationships with clients, families, and interdisciplinary teams
