RN Clinical Manager, Home Care

📋 Description

• Lead, coach, mentor, and support caregivers and other field employees who provide in-home services.
• Conduct client assessments, supervisory visits, monitoring visits, and follow-up visits in the home.
• Create, review, and update individualized care plans based on client needs, preferences, routines
• Help ensure clients receive appropriate support with activities of daily living, safety, mobility
• Educate clients, families, caregivers, and field employees on safety, fall prevention, infection
• Communicate care plans, service updates, and client preferences clearly to caregivers, families
