RN Clinical Manager, Home Care

📋 Description

• Lead, mentor, and support a team of RNs and LPNs providing skilled nursing care in clients' homes
• Conduct in-home client assessments and supervisory visits for adult and pediatric clients across
• Develop, implement, review, and update individualized plans of care in partnership with physicians
• Provide clinical oversight, coaching, and support for field nurses managing complex cases
• Conduct competency evaluations, observe care in the home, and provide timely feedback to field staff
• Facilitate orientation and client-specific clinical training for newly hired and tenured nurses

🎯 Requirements

• Current, active New Jersey Registered Nurse (RN) license
• Minimum of 2 years of RN experience
• Strong clinical assessment, organization, communication, and problem-solving skills
• Ability to provide clinical leadership and support in a home-based care environment
• Ability to travel throughout the service area for client visits, staff support, and office needs
