RN Clinical Manager, Part- Time

📋 Description

• Provide clinical oversight for clients enrolled in state-funded Medicaid waiver and personal care
• Conduct client assessments and develop individualized care plans aligned with client needs and
• Supervise, support, and provide clinical guidance to Home Health Aides and Personal Care Assistants
• Ensure care delivery aligns with approved services, authorized care hours, state program
• Coordinate with state case managers, Medicaid managed care organizations, families, caregivers, and
• Support client reauthorization processes and help ensure care hours remain aligned with approved

🎯 Requirements

• Current Registered Nurse (RN) license in good standing in New Jersey
• Minimum of 2 years of RN experience
• Strong organization, documentation, communication, and problem-solving skills
• Ability to manage state program requirements, client needs, and competing priorities
• Ability to provide clinical leadership and support to caregivers in a community-based setting
• Comfort using computer systems for documentation, communication, and care coordination
