📋 Description

• Lead field staff delivering home health per BAYADA policies and regulations.
• Coordinate client care, referrals, and case management; advocate for clients.
• Oversee all services; review clinical documentation from start to discharge.
• Work with Medicare Care Management, Managed Care, and Finance on coding and authorizations.
• Field staff supervision, coaching, and performance evaluations.
• Ensure productivity and quality measures are met.

🎯 Requirements

• Education: Graduate of an accredited nursing program.
• Licensure: Current RN license in CO in good standing.
• Experience: Minimum 2 years nursing; home health preferred; supervisory exp.
• Clinical: Home-based practice; assessment, care planning, case management.
• Interpersonal: Strong communication and team-building; mentor others.
• Compliance: Documentation accuracy and regulatory compliance knowledge.
