📋 Description

• Visit geriatric clients throughout Charlotte, NC and surrounding areas to conduct assessments and
• Develop and oversee individualized client care plans.
• Coordinate communication between clients, families, field staff, office staff, and clinicians.
• Supervise, coach, and support Home Health Aides in the field.
• Facilitate new-hire orientation for Home Health Aides and Certified Nursing Assistants (CNAs).
• Participate in employee training and in-service education to help field staff provide exceptional

🎯 Requirements

• Current Registered Nurse (RN) license.
• 2+ years of professional RN experience required.
• Home care experience preferred.
• Previous supervisory or Nurse Manager experience preferred.
• A passion for supporting clients in their homes and developing caregivers.
• Strong communication, organization, and leadership skills.
