RN Nurse Manager, Home Care

📋 Description

• Conduct in-home client assessments and ongoing reassessments as RN Clinical Manager
• Develop, review, and update individualized care plans as the assigned RN leader
• Provide clinical oversight for clients served through Assistive Care State Programs
• Complete required RN documentation, clinical notes, and care coordination activities
• Partner with the office team to ensure services align with care plans and state program requirements
• Help create a smooth, positive experience for clients who want to remain safely at home

🎯 Requirements

• Current Registered Nurse license
• 2+ years RN experience with BSN; 3+ years with ADN
• Strong clinical judgment and communication skills
• Ability to complete assessments, care plans, and follow-up
• Home care experience or case management background
• Passion for helping clients safely remain at home with independence
