📋 Description

• Lead, coach, and support field staff to ensure care quality.
• Conduct client assessments and care planning.
• Manage clinical care and case management across clients.
• Provide clinical oversight and staff development.
• Review documentation and provide timely coaching.
• Ensure continuity of care for adult and pediatric clients.

🎯 Requirements

• Current Minnesota RN license in good standing.
• Minimum 2 years nursing experience; home health/pediatrics preferred.
• Supervising or mentoring clinicians in home health/pediatrics.
• Strong clinical assessment for complex pediatric/adult cases.
• Trach/vent and tube feeding experience in home health preferred.
• Demonstrated leadership, organization, communication skills.
