RN Pediatric Nurse Case Manager

📋 Description

• Lead field staff to ensure quality client care and policy adherence.
• Conduct client assessments and care planning.
• Oversee clinical care and case management for client populations.
• Provide clinical oversight and development of field staff.
• Review field documentation and coach performance with timely follow-up.
• Support continuity of care for adult and pediatric clients.

🎯 Requirements

• Current Minnesota RN license in good standing.
• Minimum 2 years nursing experience; home health preferred.
• Experience supervising/mentoring clinicians in home care or pediatrics.
• Strong clinical assessment for complex pediatric/adult cases.
• Experience with medically fragile patients incl. trach/vent, seizures, tubes.
• Demonstrated leadership, organization, and communication skills.
