📋 Description

• Plan and manage complex care coordination cases to support timely patient access, treatment
• Conduct patient and case assessments, contribute to care planning, and facilitate transitions of

🎯 Requirements

• Bachelor's degree, registered nursing qualification, graduate-level education, or equivalent
• Approximately 5 years of nursing or relevant clinical experience, with at least 2 years of
• Experience in healthcare and case management, medical practices, hospitals, insurance, or
• French and English bilingualism is preferred.
• Strong patient-focused mindset with excellent customer service and interpersonal skills.
