Senior Habilitation Manager

📋 Description

• Serve as internal case manager and advocate for Habilitation clients
• Lead Person-Centered Planning and ISP processes
• Coordinate with clients, families, Case Managers, and care teams to set goals
• Conduct regular visits to monitor progress per ISP and service definitions
• Coach and supervise Habilitation Technicians per policies and state regulations
• Support client-specific orientations and contribute to quality improvement

🎯 Requirements

• Bachelor's degree required
• Minimum of 2 years post-degree experience with I/DD
• Supervisory or leadership experience preferred
• Strong understanding of person-centered planning and ISP
• Ability to collaborate with clients, families, and multidisciplinary teams
• Passion for supporting individuals with I/DD and promoting independence
