📋 Description

• Making home visits to clients in designated territories.
• Assess social and emotional factors related to clients' health problems.
• Provide clinical social evaluations and plan interventions.
• Identify appropriate community resource referrals to address needs of patient and family.
• Advocate for client and family to navigate the community system.
• Document observations, interventions, and evaluations on a touch pad tablet.

🎯 Requirements

• Licensed by South Carolina Board of Social Work Examiners at LMSW or LISW.
• MSW with at least 2 years of community-based experience.
• CSWE accreditation evidenced by transcript or degree.
• Home care experience a plus.
• English language skills (read, write, speak).
• Independent worker with strong time-management and interpersonal skills.
