Speech Language Pathologist, Full-time, Home Health

📋 Description

• Make home visits to clients in designated territories.
• Perform evaluations to diagnose and treat speech, language, or swallowing issues.
• Develop a treatment plan based on evaluation, orders, and goals.
• Continuously assess and revise the care plan with the multidisciplinary team.
• Educate patients and families in rehabilitative care to promote safety and independence.
• Document observations, interventions, and evaluations via a touch pad tablet.

🎯 Requirements

• Current DE Speech-Language Pathologist license.
• ASHA CCC in Speech-Language Pathology, or equivalent.
• Equivalent education and pending ASHA exam for certification.
• In process of meeting supervised experience for certification.
• Solid computer skills; EMR experience preferred.
