Speech Language Pathologist, Home Health

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform thorough evaluations to diagnose and treat speech, language, communication, or swallowing difficulties.
• Develop a treatment plan based on evaluation, physician orders, and goals.
• Continuously assess and revise the speech therapy care plan with BAYADA clinicians in the multidisciplinary team.
• Educate patients and families on rehabilitative care to promote health, safety, and independence.
• Accurately document observations and interventions using a state-of-the-art touch pad tablet.

🎯 Requirements

• Current NJ license as a Speech Language Pathologist.
• ASHA CCC-SLP or equivalent credential.
• Eligible to take ASHA exam or pursuing certification.
• Completed education/work requirements for certification.
• Solid computer skills; EMR experience preferred.
• Prior home care experience encouraged.
