Speech Language Pathologist, Home Health

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform evaluations to diagnose and treat speech, language, or swallowing issues.
• Develop a treatment plan based on evaluations, orders, and goals.
• Continuously assess and revise the care plan with the multidisciplinary team.
• Educate patients and families on rehabilitative care for health and independence.
• Document observations and interventions with a state-of-the-art tablet.

🎯 Requirements

• A current license as a Speech Language Pathologist in MD.
• CCC in Speech from ASHA, or equivalent qualification.
• Equivalents of educational requirements and work experience toward certification, awaiting ASHA exam.
• Complete education and supervised work experience toward certification.
• Solid computer skills; EMR experience preferred.
