Speech Language Pathologist, Home Health

📋 Description

• Make home visits to clients in designated geographic territories.
• Evaluate and treat clients with speech, language, or swallowing needs.
• Develop a treatment plan based on evaluation, physician's orders, and goals.
• Continually assess and revise the speech therapy care plan; participate in multidisciplinary care plans.
• Educate and instruct patients, family members, or care reps in rehabilitative care to promote health, safety, and independence.
• Accurately document observations and interventions using a touch pad tablet.

🎯 Requirements

• A current Maine license as a Speech Language Pathologist.
• ASHA CCC-SLP or equivalent qualifications.
• Eligibility to take the ASHA exam for certification.
• In the process of accumulating supervised experience for certification.
• Solid computer skills; EMR experience preferred.
