Speech Language Pathologist, Home Health

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform evaluations to assess and treat speech, language, or swallowing difficulties.
• Develop treatment plans based on evaluations, orders, and goals.
• Continuously assess and revise care plans with the multidisciplinary team.
• Educate patients and families on rehabilitative care for safety and independence.
• Document care observations and interventions on a touch pad tablet.

🎯 Requirements

• Current VA license as Speech Language Pathologist.
• CCC-SLP from ASHA, or equivalent.
• Education and work exp toward ASHA certification, including awaiting exam.
• In process of completing requirements for certification.
• Strong computer skills; EMR experience preferred.
