Conduct home visits to clients in designated geographic territories
Perform evaluations to assess, diagnose, and provide therapeutic interventions for speech
Develop treatment plans based on evaluation, physician orders, and recommended goals
Continually assess and revise speech therapy care plans, participating in multidisciplinary care
Educate and instruct patients, family members, or representatives in rehabilitative care and
Accurately document observations, interventions, and evaluations using a touch pad tablet

🎯 Requirements

Current SLP license in Vermont
Certificate of Clinical Competence in Speech from ASHA (or equivalent education/work experience)
Solid computer skills
Prior experience with EMR preferred
