Speech Language Pathologist-Home Health Visits

📋 Description

• Make home visits to clients in designated territories.
• Conduct evaluations to diagnose and treat speech, language, swallowing issues.
• Create treatment plans based on evaluations and physician orders.
• Update care plans with the multidisciplinary team.
• Educate patients and families on rehabilitative care for independence.
• Document care using a touch‑pad tablet.

🎯 Requirements

• PA license as Speech-Language Pathologist.
• ASHA CCC-SLP or equivalent; or awaiting certification.
• Complete education and supervised experience for certification.
• Strong computer skills; EMR experience preferred.
