Speech Language Pathologist, Home Health Visits

📋 Description

• Make home visits to clients in designated territories.
• Conduct evaluations to diagnose and treat speech, language, or swallowing issues.
• Develop treatment plans based on evaluation, orders, and goals.
• Update and revise the care plan with the multidisciplinary team as needed.
• Educate patients and families in rehabilitative care for health and independence.
• Document observations and interventions using a touch pad tablet.

🎯 Requirements

• Maryland license as a Speech-Language Pathologist.
• ASHA CCC-SLP or eligible for certification.
• Meet educational requirements and be eligible to take the ASHA exam.
• Complete the required supervised work experience for certification.
• Strong computer skills; EMR experience preferred.
