Speech Language Pathologist, Home Health Visits

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform evaluations to diagnose and provide therapy for speech, language, or swallowing.
• Develop treatment plans based on evaluations and physician orders.
• Assess and revise care plans with the multidisciplinary team.
• Educate patients and families in rehabilitative care and safety.
• Document care using a state-of-the-art touch pad tablet.

🎯 Requirements

• Current TN license as a Speech-Language Pathologist.
• CCC from ASHA, or equivalent credentials.
• Equivalent education and experience; awaiting ASHA exam.
• Complete education and pursuing supervised experience for certification.
• Strong computer skills; EMR experience preferred.
