Speech Language Pathologist-Home Health Visits

📋 Description

• Make home visits to clients in designated territories.
• Evaluate and diagnose speech, language, and swallowing issues.
• Develop treatment plans based on evals, orders, and goals.
• Continuously assess and revise care plan with multidisciplinary team.
• Educate patients and families on rehabilitative care and safety.
• Document interventions in EMR on a touch-pad tablet.

🎯 Requirements

• PA license in Speech-Language Pathology.
• ASHA CCC or equivalent certification/eligibility.
• Degree and supervised experience toward ASHA cert.
• In process to take ASHA exam for certification.
• Solid computer skills; EMR experience preferred.
• Prior home care experience a plus, not required.
