Speech Language Pathologist - Senior Living Visits

📋 Description

• Make home visits to clients in designated territories.
• Perform evaluations to diagnose and treat speech, language, or swallowing issues.
• Develop a treatment plan based on evaluation, orders, and goals.
• Continually assess and revise the care plan; join the multidisciplinary team.
• Educate patients and families in rehab activities for health and independence.
• Document observations and interventions using EMR on a tablet.

🎯 Requirements

• Current PA licensure as Speech Language Pathologist.
• ASHA CCC-SLP or equivalent for exam eligibility.
• Eligible for ASHA exam with required education/experience.
• Education/experience to qualify for certification or in progress.
• Solid computer skills; EMR experience preferred.
• Prior home care SLP experience a plus, but not required.
