Speech Language Pathologist, SLP - Home Health

📋 Description

• Make home visits to clients in designated territories / senior living locations
• Perform evaluations to assess and treat speech, language, or swallowing issues
• Develop a treatment plan based on evaluation, orders, and goals
• Continuously assess and revise the care plan with BAYADA clinicians
• Educate patients and families to promote health and independent living
• Document observations and interventions in an EMR system

🎯 Requirements

• Active MA Speech Language Pathologist license
• ASHA CCC-SLP or equivalent, or pursuing certification
• Degree completed; pursuing CCC / certification
• Solid computer skills; EMR experience preferred
• Home health experience a plus
