Make home visits to clients in designated geographic territories.
Perform evaluations to assess and treat speech, language, or swallowing difficulties.
Develop a treatment plan from evaluation, orders, and goals.
Monitor and revise the care plan with the multidisciplinary team.
Educate patients and families in rehabilitative care for independence.
Document care observations and interventions on a touch pad tablet.

🎯 Requirements

A current license as a Speech Language Pathologist in Colorado.
Certificate of clinical competence in speech (ASHA CCC), or equivalent and pursuing certification.
At least 1 year of SLP experience; home health experience a plus.
