Visit clients in their homes to support recovery.
Perform evaluations to diagnose and treat speech, language, or swallowing difficulties.
Develop treatment plans based on evaluation and orders.
Assess and revise care plans with the multidisciplinary team.
Educate patients and families on rehabilitative care to promote health and independence.

🎯 Requirements

Active and current SLP license.
One year of clinical experience preferred.
Home health experience welcomed; training provided.
Ability to travel locally to cases.
