Speech Language Pathologist, ST / SLP - Home Health

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform thorough evaluations to diagnose and provide therapy for speech, language, swallowing difficulties.
• Develop a treatment plan based on evaluation, physician’s orders, and goals.
• Continually assess and revise the care plan with BAYADA clinicians in the multidisciplinary team.
• Educate patients and families in rehabilitative care to promote health, safety, and independent living.
• Accurately document observations and interventions using a state-of-the-art touch pad tablet.

🎯 Requirements

• Active North Carolina SLP license.
• ASHA CCC-SLP or eligible to take the exam.
• Active pursuit or completion of required supervised experience for certification.
• Prior home care or senior living experience is a plus.
• Solid computer skills; EMR experience preferred.
