📋 Description

• Make home visits to clients in designated geographic territories.
• Evaluate and treat speech, language, and swallowing issues.
• Develop a treatment plan based on evaluation, orders, and goals.
• Continuously assess and revise the care plan with the multidisciplinary team.
• Educate patients and families in rehabilitative care to promote independence.
• Document observations and interventions using a touch pad tablet.

🎯 Requirements

• Active NC SLP license.
• ASHA CCC-SLP or equivalent; certification underway.
• Awaiting ASHA exam or certification.
• Home care or senior living experience a plus.
• Strong computer skills; EMR experience preferred.
• Supervised experience toward certification.
