• Be the direct point-of-contact for team member inquiries or concerns; respond to and resolve

• Support operational activities associated with the MDHHS Adult Home Help Program and other
• Support the patient and caregiver journey from initial engagement through enrollment and service

• Demonstrated proficiency in FreedomCare processes and systems, with the ability to learn and
