Telephonic Nurse Care Manager

📋 Description

• Conduct comprehensive telephonic and virtual nursing assessments, reviewing patient health status
• Develop, implement, and regularly adjust individualized nursing care plans based on patient needs
• Use Motivational Interviewing techniques to encourage healthy behavior changes, treatment
• Identify clinical risks, changes in patient condition, and urgent situations, escalating concerns
• Coordinate care with internal and external interdisciplinary teams, including arranging referrals
• Deliver proactive nursing interventions designed to improve clinical outcomes, reduce preventable

🎯 Requirements

• Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) from an accredited
• Active, unrestricted Registered Nurse (RN) license.
• Compact Nursing Licensure (NLC) and/or California RN license, depending on the assigned position.
• At least 5 years of broad clinical nursing experience.
• At least 3 years of experience in one or more of the following areas: care management, care
• Clinical experience managing conditions such as cardiac, pulmonary, and endocrine disorders is
